Tribe Issues Another Tribal ID Questionnaire

     TEMOAK TRIBE - For the second month in a row, the Te-Moak Tribe has issued a questionnaire about the Tribe's identification cards.
     Previously, the tribe had issued a questionaire asking tribal members their opinion about the tribal identification cards.  A previous post covering the first questionaire can be found here.  The results of the questionaire were provided to the tribal council for decision, but the council tabled the issue as reported at this post.
     The following questions are on the new ID questionaire.

     -Would you like a new Tribal Identification Card"
     -Should your Social Security Number be included on the card?
     -Should it have an issue and expiration date?
     -What blood degree would you like on the card?
     -Should it include your maiden name, if it applies?
     -Should it have the "State of Nevada" on it?
     -Should it include a Card Number to verify who the card was issued to?
     -Should it have a magnetic strip on the back?
     -What fee do you recommend for replacing your ID, if lost or stolen?

     It is unknown if previous questionnaire results and comments are still valid or if a tribal member have to submit a new questionnaire.
     All new ID questionnaires have to be submitted to the tribe by July 2.
